The Agency for Medicinal Products and Medical Devices (HALMED) is informing hereby all its customers that new Application forms for renewal of marketing authorisation and Application form for variation(s) to medicinal product dossier are accessible on the HALMED webpages in the Application forms section.
Application forms are to be submitted to HALMED in original and signed by the applicant's responsible person.
